- Referral from your General Practitioner
- Relevant x-rays and scans
- Pathology such as blood test results
- Medicare card, DVA Card, Pension Card
- Fees
- Pensioners and Disability Health Care Card patients will be bulk-billed
- All other patient fees can be partially claimed through Medicare
